Please note the following:

• X-ray and Full Medical Certificates 体检与胸部X光检查表

1. People intending to be in New Zealand for more than six months who are not from a low incidence TB country or who have spent more than three months in the past five years in a country not listed as a low incidence TB country must provide a completed Temporary Entry Chest X-ray Certificate (NZIS1096).
   (For full details and a list of low incidence TB countries please refer to the
    Health Requirements Leaflet (NZIS1121).
2. People intending to be in New Zealand for more than 12 months must provide a completed Medical and Chest X-ray Certificate (NZIS1007). 

Please provide the applicable medical certificate with the application at the time it is submitted.

1. 来自肺结核高发病率国家，计划在新西兰停留超过6个月的申请人，或者在过去5年间曾在肺结核高发病率国家停留3个月或以上的申请人必须在递交申请时提供一份填写完整的短期入境签证胸部X光检查表（NZIS 1096）。详细信息和肺结核低发病率国家名单请参照健康状况要求宣传手册（INZ 1121）。
2. 计划在新西兰停留12个月以上的申请人必须提供一份完整的体检与胸部X光检查表（NZIS 1007）。

递交申请时请同时递交相应的体检与胸部X光检查表或短期入境签证胸部X光检查表。

• Documents not written in English 非英文的申请材料

Applicants must provide a certified translation for any document not written in English e.g. if original documents are in Chinese, then you must provide English translations with the application at the time it is submitted.

所有非英文材料须附英文翻译，例如原始材料为中文，递交申请时请务必同时递交相应的英文翻译。

• Original Copies or Certified Copies of Documents 原件或公证件

Documents submitted should be either originals or certified copies. Please refer to our checklists to ensure you provide the correct documents:

Failure to provide the correct documents may lead to unnecessary delays to the processing of visa applications.
