Please note that if an applicant intends to stay in NZ for more than 6 months, they must provide a chest x-ray certificate and if the period of intended stay in New Zealand is more than 12 months full medical and chest x-ray certificates are required.
1. 来自肺结核高发病率国家,计划在新西兰停留超过6个月的申请人，或者在过去5年间曾在肺结核高发病率国家停留3个月或以上的申请人必须提供一份填写完整的短期入境签证胸部X光检查表(NZIS 1096)。详细信息和肺结核低发病率国家名单请参照健康状况要求宣传手册(INZ 1121) 。
2. 计划在新西兰停留12个月以上的申请人必须提供一份完整的体检与胸部X光检查表(NZIS 1007)。
Please note that applicants must provide a certified translation for any documents not written in English e.g. if original documents are in Chinese, then you should provide English translations with the application at the time it is submitted.
所有中文材料须有英文译本。 英文译本须由翻译公司提供。
Failure to do so has the potential to cause unnecessary delays to the processing of visa applications.
若申请人没提供所要求的文件会延误审理所需时间。
